Chaperon Health Care is a registered NDIS provider.
This means we meet the high standards required by the NDIA and can deliver services and supports to people who have their plan managed by the NDIA, as well as those who self-manage or plan managed.
We have support care, assistance and nursing care experience of over 20 years. Our professional support workers come to you in your home to offer you the support and assistance you need.

We are a group of experienced professionals dedicated to offer you and your family the best assistance and support.
We are very proud of our team that is built on experience and compassionate registered nurses, carers and support workers.
Our registered nurses are dementia trained, have over 20 years of nursing experience in Acute, Chronic, community nursing, dementia qualified with degree in dementia, rehabilitation, pain management, palliative nursing, spinal injuries, full nursing assessments and care.
As a licensed company by the NDIS and My aged care it is our responsibility to ensure that each carer/ support worker sent to your home has been pre-screened and fully qualified to give you high level of assistance and support you need. We work with only the best talent to make sure that the carer/ support worker is not only qualified to work with your specific need but also a great match as a companion in your home. In addition, our carers and support workers have been trained to respect privacy and maintain confidentiality.
Each service is led by highly experienced Registered Nurses and each team member is also specifically recruited according to previous experiences. Our support workers team members receive regular supervision and continuous training to understand the diverse and changing care needs of our clients.

Helping you get around the house with greater ease – such as transfers from bed or chair to wheelchair or toilet.

Maintain your confidence with a little help in the areas of bathing, showering, toileting, dressing, cleaning teeth, shaving and other personal hygiene and grooming activities in the comfort of your own home.

Building your skills to help you manage things like cooking, cleaning, getting dressed, using a computer or gardening more independently.

Domestic Assistance
Offering a range of domestic and personal assistance services, as well as home maintenance support to make sure your home is safe and accessible.
Shopping
Having a helper on hand can turn grocery shopping from a chore into a fun outing. We can take you to the supermarket or shopping center, and push your trolley so you are free to peruse the produce and choose your own fruit and vegetables. We can help unpack the groceries and put them away. If you prefer, we can do the shopping for you or arrange internet access so you can shop online.
Helping you get out into your neighborhood and into your local town or city by going with you from A to B on public transport.
Helping you try new things or continue existing activities whilst making friends – from hobbies, recreation, leisure, sports and arts to community programs like gardening or volunteering.
Supported Independent Living
If you are living independently, you may want a support worker to come and help for a few hours a day or week.
We can develop a plan of support to help you whenever you need. Our support workers are experienced in personal care, medication and meal preparation, and can provide a way of companionship and community access.
Respite
We provide respite care for families and support those families coping with unexpected situations – no matter where they might be.
We can arrange outings like shopping, coffee, excursions, and visits to family and friends.
Community Participation
Chaperon Service Providers can help you connect with your community in a meaningful way. Our experienced staff will assist you with whatever you want to do—whether it’s volunteering, shopping, or watching the Brisbane Broncos.

Understanding your funding & How it works
Understanding your funding
The NDIS is designed to provide you with funding for necessary supports and services to improve your everyday life. The amount of funding you receive is based on your individual plan and needs to last for the entire duration, typically 12 months after your plan started. It does not roll over into your next plan so any funding that is not used will be lost. As a result, you may receive less funding in your next plan as NDIS may see it as an indicator that your disability and the amount of support you require has changed over time.
How NDIS pricing works
The price guide for NDIS providers is designed to give you more choice of providers and get your money’s worth for the services you need. It sets the price for some supports, such as 1-to-1 assistance with self-care activities and 1-to-1 assistance to access community, social and recreational activities.
